TERMS AND CONDITIONS:
Disruption in auto payment will result in the cancellation of Renaissance Rewards membership and loss of program benefits as follows:
In the event of an auto payment decline, the practice will attempt to contact the member via phone call and email to obtain updated payment information. A second attempt to process payment will be made the following month for the previous month’s missed payment as well as the current month’s payment due. Unsuccessful processing of the second attempt will result in cancellation of membership and loss of program benefits.
Reinstatement of membership status will require payment in full of any outstanding membership fees as well as prepayment of 6 month’s subscription. The subscription can be changed to a month-to-month membership at the end of the 6 prepaid months upon written request by the member.
A 30-day written notice is required to cancel membership.
